# 申請送櫃服務

### 1.店鋪電郵&店鋪名稱：

系統已經自動為商家填入，并且在此處不能進行修改。

1）店鋪電郵可在此處進行修改，如下：

2）店鋪名稱可在此處進行修改，如下：

### 2.申請者姓名、澳門身份證號碼、電話號碼：

請商家填入申請該服務的人的姓名、身份證號碼以及手機號碼。

### 3.申請者收件地址：

請商家填入cube.mo送櫃工作人員去取貨的地址，如有綫下門店的商家，一般為自己門店的地址。

### 4.通常派送物品内容：

請商家根據自己店鋪售賣的類別填入派送物品的内容。

### 5.預計每星期單數：

請商家填入預計每星期將要送櫃的單數。

### 6.銀行名稱、賬戶名稱、賬戶號碼：

請商家填入申請該服務須轉賬使用的銀行名稱，並填入轉賬的賬戶名稱以及賬戶號碼。

如自行到辦公中心寄件 (每筆訂單MOP10)

**2.本申請會開通儲值服務及代收款項退回服務！**

申請成功後，商戶可自行開啟或關閉澳門通支付；代收澳門通款項服務（ 每筆訂單 3% )；每月結數一次，以轉賬方式退回！
